Sight Reading Table Template - for keeping scores

This is a template to use for keeping scores in sight reading albums. I have shown my table here , for November 2012 credits.

As Hellene suggested (in a message that dissappeared, that's why I wrote it again), you can write different words or signs next to good scores, or maybe you can even put stickers, I don't know, depends on the student you're keeping scores for.

Some of us may rewrite them, fortunately.
Your idea of keeping scores written in a notebook is very handy in such situations, you can recover the info anytime.
